The HERS Foundation maintains a factual database of the adverse effects of hysterectomy and oophorectomy (removal of the ovaries, ovariectomy, castration) reported to us by women who have been hysterectomized and/or castrated.
This information is used to provide periodic reports on the physical, emotional, sexual and lifestyle changes that occur in women after undergoing these operations. Your response to this questionnaire will provide valuable information that will help other women and promote understanding of these problems. Your name and address will be kept strictly confidential. Your identity will never be disclosed to anyone under any circumstances.
